- If it is The Paper Temporary ID Slip own that You have, they might not accept it and not approve your account application. They'll ask You to provide another form of National ID. You can also provide them with your Voters Card and persuade/beg them just like I did that You don't have any other form of National ID. In my case when I applied for mine, I submitted The Paper Temporary ID Slip, Voters Card and even My Birth Certificate as extra + begging just to be able to convince that I'm who my ID says I am. So You can also try what I did, and favour might fall on You.
- For Zip Code, just Google your location Zip Code and Google will tell You. Another option is to use the Zip Code of the nearest Post Office nearest To You.
- You can also use the Address of the nearest Post Office yo your House. So when your Card gets to your state, It's your Post Office that it'll be delivered to and They'll receive it on your behalf. So You just go and meet them after the due date to ask if they've delivered it.
